This appears to happen in sessions that are 96kHz or higher and using ProTools(no reports of problems with other DAW’s). The solution is to play around with ProTools playback settings page. Specifically the buffer size. Sizes of 2048 seem to give problems, whereas 1024 and lower are fine. Try using 1024 and see if that fixes it.
